I often found myself building tools for clients, adapted from other resources in my textbooks or from worksheets. I realized they might be helpful to others as well. All of the sources I used are credited on each tool.
The tools are designed to never save your work. If you fill them out, the only way to get back to or edit them is by emailing them to yourself or saving them as PDFs to refer back to later.
My website never stores your input, and I will not be able to see your responses. That means your responses are yours alone, but it also means I can’t help troubleshoot or help you retrieve any answers.
Mood Disorder Questionnaire
The MDQ is a brief screening tool for bipolar spectrum disorder. It walks through 13 yes/no questions about hypomanic or manic symptoms (feeling unusually "up," needing less sleep, racing thoughts, increased talkativeness, risky behavior, and similar experiences), then asks whether several of those symptoms tended to cluster together in the same period of time, and how much of a problem they caused.
Personality Inventory for DSM-5
A 25-item self-rated personality trait assessment across 5 domains: negative affect, detachment, antagonism, disinhibition, and psychoticism.
Level 2 Altman Self-Rating Mania Scale
The ASRM is a 5-item self-rating scale designed to assess the presence and severity of manic symptoms over the past week. It's used as a follow-up when the DSM-5-TR Level 1 Cross-Cutting Symptom Measure flags the mania domain (sleeping less but still having energy, and/or starting more projects or taking more risks than usual).
Level 1 Cross-Cutting Symptom Measure
This is an electronic version of the adult self-rated version of the DSM-5-TR Level 1 Cross-Cutting Symptom Measure. It consists of 23 questions covering 13 domains: depression, anger, mania, anxiety, somatic symptoms, suicidal ideation, psychosis, sleep problems, memory, repetitive thoughts and behaviors, dissociation, personality functioning, and substance use.
Self Compassion Scale Tracker
The Self-Compassion Scale measures how someone typically relates to themselves in difficult moments. This tool is an independent adaptation built for self-tracking and is not affiliated with or endorsed by Dr. Neff. All credits for the scale items go to Kristin Neff, 2003.
AAQ-II Flexibility Tracker
A brief self-report tracker for psychological flexibility to check in over time. Steven C. Hayes and colleagues, who developed the AAQ-II, note that movement on this measure can happen over a few weeks for some people and take a few months for others, and that a meaningful shift for one person may look different for another.
